How this is worked out. Each patient who reaches the threshold is counted once at CPT 99493, the subsequent-month code (31 or more care-manager minutes), using 2026 CMS national Medicare non-facility rates. A patient's first month bills 99492 instead, added time bills 99494, and months with 16 to 30 minutes may bill G2214; this estimate leaves those out. Patients not on Medicare are left out, because their payment depends on the payer.

No. Clinics that were on Asendar before October 28, 2026 keep their access as it is. Subscriptions apply to clinics created from that date.
