    How many minutes do I need for 99493?

    31 care-manager minutes in any later calendar month.

    When does 99494 apply?

    For each further 30 minutes beyond the first block: one unit from 86 minutes in a first month or 76 in a later month, and another every 30 minutes after.

    What if the month falls short of 99492 or 99493?

    From 16 minutes, Medicare's G2214 can apply instead. Under 16 minutes, no CoCM code is supported that month.
